You sound like a very old GA. What version of VirtualBox are you using? They should match.
Installing GA in a Windows virtual machine is simple. In the virtual machine, click the Devices tab, scroll down to Install Guest Additions CD Image... and click.
This will attach the image to the guest's optical drive. The CD should play automatically and give you a screen like this.
Click on the highlighted entry to run the GAs. Before restarting the virtual machine to install the GAs, click the Devices tab again, select Optical drives and select Remove optical drive to free the GA disk image.
